I have seen portraits like this from R. Avedon and kind of panoramas from Massimo Vitali.

As to your question: with an inordinate amount of patience. Took about 90 mins of waiting there, set up for the shot, with endless streams of people merrily carrying on in the frame as if I didn't even exist.
