Where do people get their custom size lens caps to use on barrel lenses?
I'm assuming that people would use leather back in the day and in modern times it would be 3d printed.

There's a fairly recent thread about this. There's a fellow in the UK who's 3D printing lens caps, perhaps others.

Make your own. There was a website that showed how to do it.
Plastic caps from jars of food products are not to be overlooked. Four of my larger lenses host plastic coffee can tops. Each cap measures over 4 inches (101.6mm) in diameter and cost me essentially nothing. True they are not black in color but shades of blue but I can easily live with that......
easiest option is probably still leather. get some vegetable-tanned leather, soak it in water, stretch it over the end of the lens with a layer of saran wrap under it to protect the lens, add some hair elastics or twine to hold the sides in place, let it dry, trim the edge.
You might find this site helpful if you interested in making your on caps.

Screw-in caps are slightly less convenient, but provide the best protection for the lens.
The first thing to do is to measure carefully and see whether you really need a custom-made cap:

May I also suggest
I use white Gaff tape hand torn into little strips to resize a cap
and found objects also work
but of course it is easier and cheaper to simply buy them
