Does anyone have a lens with severe chromatic aberration? Perhaps a lens that has been damaged in some way or another to cause severe misalignment? Are there lenses that are prone to this? What is the worst you have seen?

Worst lens I had was Nikon 85mm f1.8D. Also flared very easily.

i think best (or worst) would be a prism system of a 3ccd camcorder.Don't we call it CA when the intension is to focus different wavelengths to one place... not split them up (intensionally)? ;)

If it's some of the worst CA one needs (from a lens that intends to focus light), making a lens out of Schott N-SF66 (dense flint) glass should do the trick. This type of glass has a very high refractive index as well as higher dispersion than other glass types.
