Looks like a high precision copy camera,
realized on a Sinar basis:

Any idea if this is just a prototype, or if this is

From the markings in CM it looks like two different adapters for 8x10. Looks like they are designed to let you use your 4 x 5 digital backs on an 8x10 camera. One adapter looks like it shifts in 3 steps across for panorama stitches effectively 5 x 12 and the other shifts both horizontally and vertically for 4 stitches for effective 8 x 10. Looks like rubberized shutter curtains cleverly cover the open spaces.

The Japanese text says that it's a custom-built sliding adapter for a Phase One digital scanning back.
