Omega D2 focus adjustment

Yup,so the focus system keeps falling out of focus. The lens plain slides down.. I think it is loose some ware,can someone tell me how to tighten this up and ware are the tension screws?

I would check all screws on the focus knob assembly.. especially the ones that hold the rod - they may have come loose
