scaning large and medium format images

I would like to get some recommendations for scanning medium and large format transparencies and some negatives.
I mean commercial labs who do a great job without costing a fortune.

Where are you located?
What do you plan to do with the scans?
If making prints, what size prints you plan make from the scans?
Color and/or B&W?
