What is the difference between these two scanners in the real world? I want to use them for proofing and portfolio use and size (16X12" max)prints. The real scans will be done by drum scanners. Is the 4990 worth the extra for this task?

I've never used the 4490 but my recollection is that when the 4990 came out reviewers didn't find any significant differences between it and the 4490 except that the 4990 will scan 8x10 negatives. The 4990 does an excellent job with prints in the size you're talking about. I see no apparent difference between prints of that approximate size from my 4990 scans and from the (few) drum scans I've had made or from the high-end Imacon I used at the Palm Beach Photographic Workshops.

Is it correct that the Epson 4990 can not scan 6x17 panoramic slides (on 120 film), but only 6x12?

You can scan 6x17 if you have Doug Fisher's custom film holder.
The 4490 doesn't come with a 4X5 holder, please pardon my ignorance but does it have a sufficiently sized scanning light for a 4X5 frame? If I use a homemade 'holder' (using matt board or whatever) will the scanner be able to recognise and scan from it?
Next question, where the heck would I buy Anti Newton Glass in the UK of sufficient size? Can't seem to find it anywhere.
It seems that the 4490 doesn't have a large enough scanning area for 4X5 even with a custom holder, just got a great deal on an Epson 4990 so that more or less solves the problem!
the 4490 will not scan 4x5 film. I had this scanner when I was shooting 120 film, when I switched to 4x5 film I had to get a different scanner. I went for the 4990, it will scan up to 8x10 film.

Why does it matter what height the flat image is from the optical lens, if the lens is 10x8" long?
Surely the optical reading laser will pick up whatever data is on its way if the negative is directly compressed against the image.
On the other hand, most flat bed holders seem to hold the negative about 3mm or so (whatever the thickness of the plastic is) off the bed.
Does this mean that a sheet of anti-newton glass could then be used to to scan a 8 1/2 x 6 1/2 inch negative without whatever issues might crop up from having the negative sandwiched directly against the optical lens of the flatbed?
If that makes any sense to anyone other than me...
