I used an octabox, just slightly off-centre and pointed down.
B&H had a good deal on a 7-lamp fluo kit with octabox, so I jumped on it.
It's not really bright enough for film, but for digital, at 800-1000 ISO, it works well enough.
I'm a big fan of Octoboxes for portraiture. There are affordable options these days. The falloff on Miriam's face really helps focus attention on what's important.
