Custom book slipcases

Does anyone know where to get nice-looking book slipcases or clamshell boxes of a specific size to fit a particular book in quantities of around 100 ? The kind of things that come with "special/limited" edition photo books. I am located in San Jose, CA, but I don't mind taking a shipment if a distant supplier is more cost effective.
